Please complete this section of the form if you would like to receive meals from the group after the birth of your baby. Please be sure to arrange for Jennifer to be notified upon the birth of your baby, so that we can ensure that your meals will begin on time. The number of meals you receive will depend on the availability of our volunteers, but we will strive to provide at least 5 days of meals, unless you specify otherwise.
